Here at Forest Lake Family Practice we aim to provide the highest level of patient medical care.

We are ever grateful for you being with us for so long and pride ourselves in being able to provide you quality healthcare services over the years.

Please be advised that Forest Lake Family Practice has changed back to bulk billing all patients with a valid Medicare card as of November 1st 2025.

Please note: For instant rebate, patients should bring their physical debit/savings/EFTPOS card for the appointment.

Patients who do not have a valid Medicare card will be charged a fee determined by the Doctor.

We always try to keep our fees affordable. Any concern related to the payment can be discussed with our receptionist at the booking prior to the consultation.
