HOW TO FIND US
Our family friendly medical centre is joined in with Forest Lake Dental, we are the medical and dental centre in the same complex as I-MED Radiology.
Forest Lake Family Practice provides all aspects of general medical practice for patients in Forest Lake and surrounding areas. We welcome new and existing patients to our central medical centre during surgery hours, Monday through to Saturday.
Please call to book an appointment, and don’t forget to let us know at reception if a longer appointment is required.

From time to time, Forest Lake Family Practice invites patients to complete a questionnaire on their views concerning our practice and how it might be improved. These surveys are completely confidential and are intended to help us improve our comprehensive medical services.
If you are unhappy with any aspect of the medical care that you receive from this Forest Lake Family Practice, we want to know about it.
